      <image:title>About Dr. Nic - Dr. Nicholas Hedges DC, MS, CCSP®, Cert MDT</image:title>
      <image:caption>Dr. Nic is a 2019 graduate of Logan University in St Louis MO.  While pursuing his doctorate in chiropractic, he also received a master's degree in sports science and rehabilitation.  While in school, Dr. Nic was very active in clubs and as a tutor.  He was the president of the motion palpation institute club at his school, providing lectures to classmates on both adjustments and rehabilitation techniques.  He also tutored both anatomy and neuroanatomy to his classmates.  Dr. Nic also attended over 500 contact hours of continuing education coursework in adjustment techniques, dynamic neuromuscular stabilization, acupuncture, dry needling, soft tissue therapy, and more.   Before chiropractic school, Dr. Nic ran cross country and track at Adams State University.  He graduated in 2016 with a degree in sports psychology.  During his time at Adams State, he also discovered a love for the outdoors of Colorado and found a passion for rock climbing, trail running, and skiing.   When not practicing or enjoying the outdoors, Nic enjoys reading, cooking, and teaching.  Dr. Nic also writes not only for the blog on SCR3, but for academic publications and websites as well. He has written on a variety of topics ranging from chronic spine pain to how to properly rehabilitate an ankle sprain. Click on the links below to check out some of his writings. Since graduation, Dr. Nic has also continued his education to become a Certified Chiropractic Sports Physician® (CCSP®), as well as becoming attaining the MDT certification. The CCSP is an indicator of specific education and experience in working with and treating athletic populations. MDT stands for Mechanical Diagnosis and Therapy. This is a method that is designed to empower you to understand the nature of your symptoms and how they respond to exercises and movements. This is not a cookie cutter approach, and is designed to give you the specific exercises and rehabilitation to help you improve in the most cost effective manner.       <image:caption>Dr Ivo was born in Biloxi, Mississippi on Kessler Air Force Base and grew up in upstate New York. He is a 1986 summa cum laude graduate of the New York Chiropractic College. He received his post-graduate neurology diplomate from Logan Chiropractic College and became board certified in 1990. Dr Ivo works with a broad base of patients including Olympic level, professional and paraprofessional running, skiing and cycling athletes. He is an adjunct faculty member with the National School of Post-Graduate Education and is one of the 2 founding members of "The Gait Guys", a consulting group which studies and teaches about lower extremity biomechanics, rehabilitation and movement and gait related disorders.  Dr Ivo researches, publishes and PODcasts extensively. He is a board certified Chiropractic Neurologist, Chiropractic Rehabilitation Physician and certified Chiropractic Sports Physician. He lectures in the US and Canada on foot-ankle and gait biomechanics in sports, rehabilitation, dry needling and acupuncture techniques. He is co-author of the textbook, Pedographs and Gait Analysis: Clinical Applications and Pearls.   Additional Background Dr Ivo F Waerlop  attended SUNY at Albany for his undergraduate work and graduated  from the New York Chiropractic College in 1986. He then joined the Core Science faculty at the college and taught anatomy and human cadaver dissection as well as Gonstead and Diversified chiropractic techniques. It was at this time he conducted research performing a serial dissection of alar ligaments in cadavers and completed post graduate work in Sports Medicine, receiving his Certified Chiropractic Sports Physician certification. Dr Ivo then moved to Gloucester, Massachusetts where he founded Gloucester Chiropractic and Rehabilitation, a medically referral based office complete with CYBEX lab. During his time in Massachusetts, he completed additional post graduate work in rehabilitation and was accepted into the chiropractic neurology program at Logan Chiropractic College in St Louis, Missouri. Here, he was one of the fortunate doctors who studied directly under Dr Ted Carrick, and after 3 years and passing his written and oral board examinations, was awarded his Diplomate in Neurology Degree in May of 1991. At this point in time, Dr Waerlop began his post graduate teaching career, 1st in rehabilitation and later in neurology. In late 1996, Dr Ivo relocated to Summit County Colorado, continuing to teach on a post graduate level and began a private practice in Frisco at the CARE Health Group. He began teaching Anatomy and Physiology locally at the Colorado Mountain College, which he discontinued after the birth of his 1st son. In 1999, he relocated to a larger facility in Silverthorne and founded Summit Chiropractic and Rehabilitation, PC, a rehabilitation and neurology referral based practice, with an emphasis on gait and movement analysis. In 2004, he became interested in acupuncture, largely because his 150 pound German Shepherd (Atlas) was epileptic, and a Veterinary Acupuncturist from Grand Junction (Dr Shalona McFarland) was able to help him control his seizures. Dr Ivo began his studies shortly thereafter and received his acupuncture license in the summer of 2005. He then studied neuromuscular acupuncture under Dr Yun Tao Ma and oulse diagnosis and cosmetic acupuncture under Dr Martha Lucas. In August of 2006, Dr John Asthlater joined the practice and the office was moved to our new facility in the Dillon Tech Center, in Dillon. In addition to injury, rehabilitative and preventative care, the new center offers the latest in cutting edge diagnostics, inclluding digial x ray and diagnostic ultrasound, video motion analysis for both gait and bike pedal stroke, an orthotic lab, full blood testing, lactate testing, training programs, and expanded hours. When Dr Ivo isn’t seeing patients or lecturing, he enjoys spending time skiing, mountain and road biking with his wife, Lisa and his 2 sons Vander and Saxxon. His hobbies include reading, music and cooking.</image:caption>

      <image:caption>Diagnostic ultrasound is an incredible tool for looking at and assessing the integrity of soft tissues underneath the skin. Ultrasound works by using sound waves and their reflexions to get a glimpse at what is going on in the connective tissues that we cannot ordinarily see. In many cases in which diagnostic ultrasound is used like ankle sprains, shoulder pain, and knee pain; ultrasound is just as sensitive to pathology as an MRI. While it is just as effective is assessing certain condition, it is a fraction of the price.       <image:title>Low Back Pain Condition - Disc Herniation</image:title>
      <image:caption>Another common culprit in low back pain is the intervertebral disc. The intervertebral discs are in between the bones of the vertebral column. They have two different components to them; the annulus fibrosis and the nucleus pulposus. The annulus runs around the disc and is very tough. The nucleus pulposis in the inside of the disc. The Nucleus pulposus houses a gelatinous material which aids our body’s natural shock absorption mechanism. If the annulus fibrosis is overloaded, then the gelatinous material of the nucleus pulposus can herniate through it. This can sometimes be the cause of acute pain both in the back and running down the leg as pressure is placed on nerve roots emanating from the lumbar spine. It is important to recognize the most of these instances do not require drugs or surgery to become better. In fact, with proper education, therapies, and active rehabilitation, the majority of disc herniations will heal and resolve on their own.</image:caption>

      <image:title>Knee injury - Knee Arthritis</image:title>
      <image:caption>Previously thought of as a solely structural and “wear and tear” type issue, we now know that many factors go into making an arthritic knee painful. These factors range from overall inflammation, to simple muscle weakness. A large study of over 46,000 people found that weakness in the knee extensors (quads) was a risk factor for the development of knee osteoarthritis (Øiestad et al. 2021). This is definitely one of the cases in which preventative measures for knee arthritis really include resistance training of the knee. In the treatment of knee arthritis itself, contrary to what some may believe, movement can be exceedingly beneficial whether that be walking, squatting, lunging, or running. Long term management with over the counter anti inflammatories and injections are potentially dangerous in that they may decrease what cartilage is left in an unhappy knee (Perry et al. 2021; Singh et al. 2021; Orchard 2020).  Some things that may be beneficial include supplementation with collagen peptides and also movement assessments to look for a direction of preference (Zdzieblik et al. 2017; Rosedale et al. 2014).</image:caption>

      <image:title>Knee injury - Patellofemoral Pain syndrome.</image:title>
      <image:caption>This is an interesting pain manifestation behind the kneecap which causes pain while squatting, going up stairs, and can even present after sitting for long periods of time. We used to think that the patella was tracking incorrectly along the condyles of the knee and that these “tracking issues” led to abnormal wear and tear and then pain the back of the kneecap. Unfortunately, tracking the patella is easier said than done, and when we do track it, it seems as though all tracking manners are present even in people without pain in their patella. A few things that were found to be very helpful include some manual therapies along with active exercises. These HAVE to be done together because performing one of these alone is not superior to doing both. The manual therapies that can be helpful to the recovering knee include soft tissue therapies like pin and stretch of the muscles surrounding the knee, as well as acupuncture. The rehabilitation of this injury has also changed significantly in the past 5 years. We once thought that we needed to target the tracking issue of the knee and support it with tape in controlled ranges of motion. However, this has been found to be less effective than a graded loading program to the extensors and stabilizers of the knee. Ironically many of the major stabilizers of the knee are located up in the hip. The muscles in the outside and back of the hips are major players of knee stability, and can be easily strengthened with specific exercises that do not even require a resistance band or tape ((Willy et al. 2019; Winters et al. 2020)</image:caption>

      <image:caption>some ligament injuries can (and should) be treated conservatively and without surgery, while some will likely need surgery to repair important structures that provide the knee with some crucial aspects of stability. The four major ligaments of the knee that provide really important stability for skiers are the Anterior cruciate ligament (ACL), Posterior Cruciate ligament (PCL), Medial collateral ligament (MCL) and the Lateral Collateral ligament (LCL).  One of the most important things that we do in our office regarding knee pain is a thorough assessment and evaluation if you are a good candidate for our care. Complete tears of one of these ligaments compromises the structural integrity of the knee and leads to significant laxity, which can then lead to damage in other areas and tissues of the knee, which can predispose you to arthritis and joint damage. Luckily, orthopedic tests as well as thorough history taking (both of which can be performed by trained sports chiropractors) can be effective in figuring out if a referral to a surgeon or for imaging is necessary. Complete tears of the ACL and other ligaments are the best candidates for surgery, but minor tears and sprains of ligaments like the MCL and LCL are very effectively managed with non surgical means. The best way to manage these is through a combination of manual techniques and guided rehabilitation which can be performed by sports chiropractors and physical therapists.</image:caption>

Like an overstretched rubber band, the capsule is then more loose and predisposes the individual to further dislocations. The types of loads that can commonly cause tendon pain are high intensity dynamic loads, which is why athletes such as runners, jumpers, and gymnasts have higher rates of tendinopathy than powerlifters and athletes that use more higher loads, at a slower rate. One of the ways we have to modify activities is that we need to begin incorporating heavy, slow loads, and initially avoid the loads that may have brought about the tendon changes. If you are consistently experiencing a recurrence in your tendon pain, it is likely that you are using incorrect loads for your rehab. Start doing the correct type of loads consistently, and over time the tendon and muscle will adapt to increase your load tolerance as you get back to normal activities.</image:caption>

      <image:caption>A treatment that used to be quite commonplace for tendon pain was injection of corticosteroid medication. This was especially prevalent back when we really thought that inflammation was the big factor in the development and chronification of tendon pain, however this is not the case.  Injecting steroids into tendons may seem like a good idea at first, but this does nothing for benefiting the structure of the tendon, nor does it produce long lasting changes in tendon capacity. Sometimes injections can have a short term benefit in reducing pain, but over time, and especially if injections are repeated, these therapies can degrade the structure of the tendon and potentially make it more susceptible to rupture.  The best treatment for chronic tendon pain is going to be a consistently followed loading program. Injection therapy should not even be considered until multiple months of conservative management and loading have failed.</image:caption>

Creating compressive loads on tendons can exacerbate and worsen our tendon pain. Passive stretching does not actively load the tendon to create lasting change. Once tendon pathology is visible on imaging, it is very difficult to change the structure back to normal. However, this does not mean that we can’t return to being fully functional. What we do know is that the vast majority of individuals with tendon pathology still have enough good tendon to be fully functional and meet their goals. What this means is that we shouldn’t focus on changing the altered portions of the tendon, but focus on increasing the capacity of the healthy portions of the tendons.</image:caption>

      <image:caption>It makes a lot of sense to worry about painful structures breaking, but in the case of tendons, it is not something we should spend a whole lot of time worrying about. In fact one of the first loading programs was actually performed as a patient was actively attempting to rupture their own achilles! This patient had debilitating achilles tendon pain, and their doctor refused to do surgery because the tendon was not ruptured. So this patient attempted to rupture their own achilles by performing repeated heel drops. To their surprise they did not rupture their achilles, but they actually started having less pain! This was the first loading program ever (named the Alfredson protocol in case you’re interested) used in treating tendon pain and is now recognized as the best practice model in treating tendons (especially after refining the loading programs based on athlete needs).</image:caption>

That may sound like a crushing blow, but that is just the reality of the matter.We have to recognize that gimmick treatments and quick fixes are not the best answer to tendon pain. Now during that time period, we will be loading the tendon and using sound programming to manage loads properly, and in most cases sport activities can be resumed much earlier than 6-8 months. The important thing to know is that it is not a fast process, but it is a process that you have to trust in order to get back to a full, or maybe even better capacity.</image:caption>
